Arent you gonna start a poll with those numbers to select from? Do u know how to start a poll?
Also the poll is too broad. You gotta be more specific and ask great in what area?

It's not the country of America any longer, the US went bankrupt March 9th 1933 and was taken over by the commies who are the central bankers..... This is the world wide take over by a group of people armed with pens and the sheep are to dumb to figure it out.

American also became incorporated in 1870 and no longer was a country, but a corporation.
America embodies the good, the bad and the ugly. So it's hard to give it a rating. However if you got specific, and asked about the dating and social life there, I'd give it very low marks of course. Same low rating for mental health there too. But for other stuff like consumer choices, customer service, clean streets, clean air, beautiful nature, etc. I'd give it high marks.

Difficult to rate, but I am not from USA and would say, it is not really a good place (4) but also not really a below average place (2)
in general.
I would vote for 3.
It is a wide country and as US-citizen you have a choice - you can settle down wherever you find a place which fits you. This is good (4)
On the other side social life is really bad. The behavior of many women is terrible.
Legally seen as a man if you prefer family life, better move away.
Compared to many countries worldwide, social life in the States is (2) below average, for sure.

I rate it a 5 for wealth-building and opportunity.
I rate it a 3 for quality of government and government services.
I rate it a 2 for the overall physical appearance of the population.
I rate it a 2 for its unapologetically poor treatment of men both socially and legally.
I rate it a 2 for quality and sophistication of custom(s) and culture(s).
I rate it a 1 for its promotion of hyper-consumption and personal debt slavery.
I rate it a 1 for quality of women.

Using Contrain Exptriate's format. Here is mine.
I rate it a 5 for wealth-building and opportunity.
I rate it a 4 for safety.
I rate it a 2.5 for the overall physical appearance of the population (higher for men than women).
I rate it a 2 for Healthy Lifestyle
I rate it a 2 for its promotion of hyper-consumption and personal debt slavery.
I rate it a 1 for opportunities for romance with a nice female.
The main reason immigrants come to America is to make money. It certainly isn't for the social life. If you are skilled in a desired profession you can make money. If not you'll probably rate "wealth building and opportunity" as much lower. America works best for the highly skilled and hard working. It makes little difference how successful you are financially, because it doesn't transfer to the romance arena. Women prefer work over romance. Hyper-consumption as a replacement for relationships with women and most men don't care much about making friends either.
